Compare travel-claim charges with local reference evidence

Normalize documented treatment charges and compare them with appropriately matched local reference cases, preparing questions for a human claims specialist.

5 implementation stepsIntermediate

How this works

Prepare a factual charge comparison after claim intake has established which documents belong to the case. This workflow concentrates on itemized receipts, service location, date, currency and reference comparability. It helps a claims reviewer identify charges that need explanation without deciding whether the treatment was necessary or the policy must pay.

The source documents may include unfamiliar service descriptions and different billing bundles. Preserve the original wording alongside normalized labels. Retrieve only authorized historical cases or approved price references. A numerical difference is a review question, not proof of overbilling, fraud, clinical inadequacy or an excluded condition.

Before you start

Permitted claim scope

Confirm the claim owner and minimum records needed for billing review; restrict unrelated health details.

Comparison policy

Obtain approved dimensions for comparable services, locations, dates, facility types and billing inclusions.

Cost references

Use a dated, permission-appropriate reference collection and a documented currency-conversion convention if required.

Prepare the inputs

claim_charge_documents
Required

Itemized bills and receipts

Retain document IDs, original language, dates and unreadable fields.

service_context
Required

Location, service date and documented billing context

Do not infer clinical severity or diagnoses from a price.

reference_charges
Required

Approved comparable records

Each reference needs provenance, date, currency, units and known inclusions.

comparison_rules
Required

Versioned review criteria

Specify allowed conversions, age of references and conditions that prevent comparison.

Implementation, step by step

  1. Extract the billed lines

    • Capture service description, quantity, unit amount, tax, total and currency from each original.
    • Flag deposits, credits and duplicate copies so they are not counted twice.

Checkpoint: The line register reconciles with the readable bill totals.

  2. Normalize without changing meaning

    • Create a separate comparison label while preserving the original service wording.
    • Separate bundled procedures from individual lines and retain uncertainty about included items.

Checkpoint: A reviewer can reverse every normalized label to the original text.

  3. Select comparable evidence

    • Retrieve records matching the approved geography, time period, facility setting and billing unit.
    • Exclude weak matches explicitly; a sparse reference set is reported as insufficient evidence.

Checkpoint: Each accepted comparison has a stated match basis and source.

  4. Calculate transparent differences

    • Use a spreadsheet or code for totals and authorized currency conversions.
    • Display source values, conversion date and calculation separately; do not silently fill missing charges with zero.

Checkpoint: The reviewer can reproduce every numeric variance.

  5. Prepare the review questions

    • Explain differences that may come from bundles, timing, taxes or missing documentation.
    • Route uncertain service equivalence to the appropriate specialist and draft precise requests for itemization.

Checkpoint: The packet contains questions and comparison evidence, with no payment or clinical decision.

Decisions along the way

No suitable local reference exists

Return insufficient comparison evidence; do not substitute an unrelated country’s price.

Service names appear similar but billing scope differs

Keep both records and request clarification before calculating an apparent excess.

Check the result

  • Verify bill totals, credits and quantities with deterministic calculations.
  • Confirm comparison records do not expose other claimants’ identities.
  • Check that currency and bundled-service differences remain visible.
  • Ensure no recommendation diagnoses a person, denies coverage or labels a claimant fraudulent.

When something goes wrong

Translation leaves a service ambiguous

Retain the original and route the term for specialist translation.

Reference dates are stale

Label the limitation and request a current approved comparison instead of extrapolating.

A worked example

SYNTHETIC EXAMPLE · NOT A REPORTED BUSINESS RESULT

Request

Synthetic: prepare review of a documented charge.

Inputs

A bill lists a bundled visit at 240 units. Two local references list consultation-only prices, while the bundle’s laboratory inclusions are unclear.

Expected result

Show the bill and references but withhold a direct excess calculation. Ask for the bundle breakdown and a like-for-like local reference.

Run a small pilot

Start small

Replay closed billing reviews with approved reference matches and deliberately poor matches.

Record the baseline

Record wrong matches, missing credits and preparation time in manual review.

Agree success criteria

Require reviewer agreement on accepted comparables and complete numeric provenance.

Stop or revise

Stop comparison when permissions, currency basis or service equivalence are unresolved.

Keep the scope clear

  • Clinical appropriateness, policy interpretation and payment decisions belong to qualified humans.
  • No insurer record is changed and no claimant message is sent by producing this packet.

Check before using

A human claims specialist decides coverage, payment and further medical review; the assistant does not judge clinical necessity or recommend treatment.
